12/3/2023 EnchantaVaughn Approved 1.Behavior: QMHP-C arrived to the home of Ashiyah. Upon arrival to the home setting, Ashiyah appeared to be in a positive mood evident by her facial expressions such as a smile on the face. Ashiyah appeared to be well groomed and prepared for the session. QMHP-C engaged Ashiyah in a discussion to determine if there were any new behaviors since the last session. Ashiyah reported there were no behaviors she continues to be the person that she is at which mother reports not respecting those who are in authority and having to be redirected 3-4 times with 30 minutes in duration on a daily basis. 2. Behavior: Ashiyah participated in the session and did not need any redirection from QMHP-C. Ashiyah presented on task behaviors and presented frustrating behaviors at times when she was discussing the difference between her and the rest of her siblings at which mother/ Ashiyah reports fussing with them on a daily basis for about 10-15 minutes in duration. 1. Intervention: QMHP-C began the session by inquiring about Ashiyahâs current thoughts, feelings and emotions. QMHP-C educated Ashiyah on appropriate ways to communicate with her father and her mother about her likes and dislikes. QMHP-C engaged Ashiyah in a role-playing exercise to assist her with being able to strengthen her communication skills without the use of verbal aggression. QMHP-C educated Ashiyah on an old saying, âHe who controls you angers youâ. QMHP-C provided Ashiyah with a worksheet on anger QMHP-C processed with Ashiyah about how anger can cause us to lose our behavior and how we can calm our anger and stay in control. 2. Intervention: QMHP-C engaged Ashiyah in exercises demonstrating how to relax and calm down (breathing deeply, muscle relaxing, and how to combine the two). QMHP-C instructed Ashiyah to identify ways in which she handles her anger in positive ways. Ashiyah was engaged in a role-play where anger was created. After processing the role-play Ashiyah was instructed to discuss how controlling her anger could have changed the situation. 1. Response: Ashiyah appeared to be receptive to QMHP-Csâ interventions and services that were provided to her during the session. Ashiyah indicated that what she tries to do is live by what little bit hid mother taught her when she was growing up. Ashiyah discussed with QMHP-C, her mother needs to stop thinking that she is going to turn out like her or like or sister because she is different from both of them. Ashiyah was able to identify ways in which she positively handles her anger. 2. Response: Ashiyah explained ways in which she has not handled her anger appropriately such as getting into fights with others in the neighborhood, school, and community. After role play and she was able to provide ways in which she could have handled her anger more appropriately in past situations. Ashiyah appeared to be pleased with the role-play and was able to process the role-play appropriately without becoming angry during the session. QMHP-C Interpretation of clientâs progress towards todayâs objectives: Ashiyah continues to struggle with solving conflict. QMHP-C Plan of Action: VACCD staff will continue to encourage Ashiyah to communicate through journaling her feelings and thoughts when feeling sad or depressed 1-2 times weekly.
